1. Click "Pay Now."
2. Enter the description of the payment in the “Description” box (for example, “Copayment for [your name or initials]”).
3. Enter the total payment in the “Price per item” box.
4. Click "Continue."
5. If you have a PayPal account, you can log in and continue your payment from there.
6. If you do not have a PayPal account or do not wish to log in, scroll down and click on “Pay with Debit or Credit Card.”
7. Enter the email address, phone number, card information, and address associated with your card payment.
8. Click “Continue.”
9. Your card will be charged, and you will receive a payment receipt to the email address you entered.
